About the position

Do you want to be a part of a transformational/pioneer nursing team model at St. Peters Health Partners? Come join our team, as a Virtual Registered Nurse (RN) that will serve as a support to our bedside care teams across our network of hospitals in the capital region, in an On-Site Virtual setting, located within our Albany Memorial Campus on Northern Blvd in Albany! Utilizing technology, you will support patients and bedside/floor nurses via in-room TV monitors allowing patients to have access to their care team at any time of the day. As a Virtual Registered Nurse (RN) on the care team you will be a part of/responsible for collaborating, communication & facilitation of optimal provision of care through a computerized &/or digital platform supporting daily management of care. You will be responsible for the assessment, planning, implementation, coordination, monitoring & evaluation of the patient's plan of care from admission to discharge. You will utilize your clinical knowledge, critical thinking skills & the principles of case management & adult education to coordinate, implement & support the plan of care. Ensuring a seamless, effective, efficient transition of care across the continuum is crucial. You will also monitor quality metrics specific to the department & ensure complete & accurate documentation in the patient record. In this role, you will perform the roles of mentor, clinician, educator, consultant/collaborator, and navigator.

Responsibilities

Requirements

  • New York State Valid Nursing Licenses from an accredited professional School of Nursing.
  • Minimum of 8 years in healthcare/critical care as a Registered Nurse.
  • Experience with Electronic Medical Records.
  • Current or recent bedside RN experience.
