About the position

The Virtual Registered Nurse (RN) at Albany Memorial Hospital is responsible for providing optimal patient care through a digital platform. This role involves assessing, planning, implementing, coordinating, monitoring, and evaluating patient care from admission to discharge, ensuring effective transitions across the continuum of care. The Virtual RN will utilize clinical knowledge and critical thinking skills to support patient care and mentor other staff members.

Responsibilities

• Collaborate and communicate effectively to facilitate optimal patient care through digital platforms.
• Assess, plan, implement, coordinate, monitor, and evaluate the patient's plan of care from admission to discharge.
• Utilize clinical knowledge and critical thinking skills to support the plan of care.
• Ensure seamless transitions of care across the continuum.
• Monitor quality metrics specific to the department and ensure accurate documentation in patient records.
• Perform roles of mentor, clinician, educator, consultant/collaborator, and navigator.

Requirements

• Valid New York State Nursing License from an accredited professional School of Nursing.
• Minimum of 8 years of experience in healthcare/critical care as a Registered Nurse.
• Experience with Electronic Medical Records.
• Current or recent bedside RN experience.

Nice-to-haves

• Bachelor of Science in Nursing (BSN) preferred.
