About the position

The Virtual Registered Nurse (RN) at Trinity Health Michigan is a part-time position responsible for collaborating with the Nursing Care Team to facilitate optimal patient care through digital platforms. This role involves managing the patient's care from admission to discharge, ensuring effective communication and coordination of care, and monitoring quality metrics. The RN will utilize clinical knowledge and critical thinking skills to support the patient's plan of care, ensuring seamless transitions across the healthcare continuum.

Responsibilities

• Collaborate with the Nursing Care Team to provide optimal patient care through digital platforms.
• Assess, plan, implement, coordinate, monitor, and evaluate the patient's plan of care from admission to discharge.
• Utilize clinical knowledge and critical thinking skills in case management and adult education.
• Monitor quality metrics specific to the department and ensure accurate documentation in the patient record.
• Facilitate effective communication and coordination of care across the healthcare continuum.

Requirements

• Valid RN licensure within the State of Michigan.
• Baccalaureate Degree in the Science of Nursing (BSN) from an accredited school of nursing is preferred.
• Minimum eight (8) years of experience in health condition management, including adult condition management.
• Experience with Electronic Medical Records (EMR).

Nice-to-haves

• Specialty credentialing and educational degree according to clinical nursing practice specialty area is preferred.
