About the position

The Visiting LVN for Home Health Agency role involves providing nursing services in a home care setting, supporting the Director of Nursing and nursing staff in delivering high-quality healthcare. The position requires a clinically experienced nurse who can perform patient care assessments and various medically related tasks, ensuring compliance with healthcare regulations while maintaining effective communication with patients.

Responsibilities

• Providing nurse visits to home health patients (home visits).
• Caring for ill, injured, or convalescing patients by implementing the plan of care initiated by RN.
• Providing basic patient care and treatment.
• Documenting consistently with the plan of care.
• Communicating with patients to schedule visits.
• Educating patients on self-care during non-visit days.
• Demonstrating knowledge of regulatory requirements.

Requirements

• Current CA LVN License (Required)
• BLS from the American Heart Association and/or the American Red Cross (Required)
• NSO Nursing License Insurance (Required)
• 1 year of experience as a Visiting LVN for Home Health (Required)
• 1 year of nursing experience (Required)

Nice-to-haves

• IV/Phlebotomy Certified (Preferred)
