About the position

The Waiver Case Manager position at Country Services, Inc. involves providing individualized support to children and adults with disabilities, focusing on person-centered thinking, self-determination, and empowerment. This role requires managing a caseload of approximately 45-55 individuals, conducting face-to-face meetings, and coordinating support services to enhance independence. The position is primarily remote, with community meetings required, and offers opportunities for advancement within a supportive work environment.

Responsibilities

  • Manage a caseload of approximately 45 - 55 individuals.
  • Meet face-to-face with clients a minimum of twice per year at various locations.
  • Assess needs and plan support to enhance independence.
  • Coordinate funding for support services and make referrals to service providers.
  • Create service agreements for payment to service providers.
  • Write annual Support Plans detailing individual hopes, dreams, and outcomes.
  • Complete case notes in SSIS for billing documentation.
  • Assess need for crisis services and complete referrals as needed.

Requirements

  • Bachelor's degree in social work and LSW, or a related field such as Sociology, Psychology, or Human Services.
  • Experience working in human/social services.
  • Valid driver's license and reliable vehicle with proof of auto insurance.
  • Background clearance.
  • Excellent organizational skills and ability to track timelines.

Nice-to-haves

  • Experience with Rule 185, DD waiver, MN Choices, Medical Assistance, and Medicare.
  • Computer competency and experience with Microsoft 365 (Outlook, Word, Excel).
