About the position

Aquinas Home Health is seeking a dynamic Registered Nurse to join their weekend team, focusing on providing home care services. This full-time position involves working three 12-hour shifts on weekends and one weekday, emphasizing patient care, community health education, and effective communication with healthcare providers and families.

Responsibilities

• Completes an initial assessment of patient and family to determine home care needs.
• Provides a complete physical assessment and history of current and previous illnesses.
• Regularly re-evaluates patient nursing needs and initiates the plan of care.
• Develops a care plan that establishes goals based on nursing diagnosis.
• Administers medications and treatments as prescribed by the physician.
• Counsels the patient and family in meeting nursing and related needs.
• Identifies discharge planning needs as part of the care plan development.
• Prepares clinical notes and updates the primary physician as necessary.

Requirements

• Registered nurse with current licensure to practice professional nursing in both MO and KS.
• Maintains a current CPR certification.
• At least one year of home health or hospice experience required.
