About the position

The Weekend Hospice, General Inpatient Nurse (RN) Case Manager is responsible for providing compassionate end-of-life care and hospice services to patients in an inpatient setting. This part-time role focuses on delivering holistic, family-centered care while collaborating with healthcare professionals to ensure optimal patient outcomes. The position requires a commitment to maintaining the dignity of dying patients and supporting their families during this critical time.

Responsibilities

• Implement care to achieve outcomes of comfortable and safe dying, self-determined life closure, and effective grieving for patients and families.
• Maintain the dignity of the dying patient while supporting their unique spiritual and cultural beliefs.
• Provide appropriate support at the time of death, including disposition of medications and notification of funeral homes according to agency policy.
• Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
• Provide referral sources with information on hospice programs and coordinate patient care for optimal transitions from the hospital to hospice care settings.
• Attend interdisciplinary team meetings for full team discussions, consultation, and evaluation of patient/family needs.

Requirements

• Bachelor's Degree in Nursing preferred or Graduate of Diploma or Associate Degree program in Nursing within the state of employment.
• Minimum 3 years inpatient clinical experience, preferably in hospice.
• Valid RN License.
• Valid driver's license and auto insurance in your name as a driver.
• Capable of all physical demands.
