About the position

The Wellness Associate II at Geisinger Health Plan is a community-facing role focused on serving CHIP and Medicaid populations. The position involves assisting members with navigation, addressing social needs, conducting biometric screenings, and providing health education through various community-based initiatives. The associate will also lead educational sessions for children and adults, both virtually and in-person, while developing and implementing wellness programs in collaboration with local facilities.

Responsibilities

  • Serve community or practice/facility-based population management initiatives including on-site programs, screening services, and fitness classes.
  • Develop and oversee health and wellness programs at the facility and surrounding community initiatives.
  • Design programs in conjunction with assigned facilities, ensuring compliance with competencies and training requirements.
  • Support regional teams and operations in the design and implementation of new products and services.
  • Develop tools to evaluate the effectiveness of wellness programs and manage outcomes.
  • Report program participation and outcomes quarterly to stakeholders.
  • Prepare and present educational information consistent with the wellness program's mission and objectives.
  • Provide on-site screening support and education as needed.
  • Represent the facility at various business and community events or coordinate events at the facility.
  • Serve as a wellness resource and oversee member communication and outreach initiatives.
  • Monitor changes in employee wellness research and standards.
  • Research and create new programs to meet population needs.
  • Support reporting needs, data management, and outcomes.
  • Assist with policies that have wellness components.
  • Learn new software applications and maintain databases.
  • Coordinate ongoing record keeping and prepare reports as requested.
  • Work closely with internal departments including Health Services, Marketing, IT, Sales, and wellness.
  • Manage calendar and promote activities in conjunction with marketing and coordination of onsite activities.
  • Participate on committees to represent and provide expertise related to the Wellness Program.
  • Provide telephonic and on-site health coaching for lifestyle management programs.

Requirements

  • Bachelor's Degree in a Healthcare Related Field (Required)
  • Minimum of 1 year of related work experience (Required)
  • Computer Literacy

  • Critical Thinking Skills
