About the position

The Wellness Coordinator at Elwyn plays a crucial role in providing therapeutic and preventative health care to children with severe behavioral health issues in either a school or residential setting. This position involves administering medications, monitoring physical and psychological health, and collaborating with an interdisciplinary team to develop and implement individualized health care plans. The Wellness Coordinator is responsible for ensuring the well-being of clients while adhering to Pennsylvania's Nurse Practice Act and Elwyn's policies.

Responsibilities

  • Administer medications and treatments as prescribed by physicians.
  • Monitor each child's physical and psychological status and facilitate medical follow-up as needed.
  • Collaborate with healthcare professionals to plan and implement therapeutic programs of care.
  • Conduct comprehensive evaluations including medical and behavioral history upon admission.
  • Develop and revise health care plans for children requiring 24/7 care.
  • Facilitate communication with community-based social service agencies and healthcare providers.
  • Educate staff and families on chronic medical conditions and emergency medication administration.
  • Maintain documentation and health records, ensuring confidentiality of medical information.

Requirements

  • Licensed to practice as a Licensed Practical Nurse (LPN) or Registered Nurse (RN).
  • Clinical experience in a behavioral health setting preferred.
  • Strong organizational and time management skills.
  • Ability to work effectively as part of a team in a fast-paced environment.
  • Excellent judgment and problem-solving skills with minimal supervision.
  • Intermediate experience with Microsoft Office applications, including Word and Outlook.
  • Current, valid driver's license with three years of driving experience.

Nice-to-haves

  • Knowledge of Electronic Health Record (EHR) / Electronic Medical Record (EMR) systems.
  • CPR Certification.
  • Medication administration experience.
  • Experience with tube feeding.
