About the position

The Wellness Director I at The Haven at San Gabriel is responsible for managing the day-to-day clinical services of the community, ensuring that residents' healthcare needs are met with respect and dignity. This role involves supervising care staff, providing leadership and coaching, and ensuring quality care as residents' healthcare needs evolve. The position requires a strong understanding of nursing regulations and the ability to manage complex healthcare scenarios within the community.

Responsibilities

  • Manage the day-to-day clinical services of the community to meet residents' healthcare needs.
  • Supervise and provide leadership to community-based licensed, certified, and non-certified care staff.
  • Assign and direct work of care staff, appraise performance, and address complaints.
  • Provide ongoing training and education to caregivers through classroom in-services and situation-specific training.
  • Supervise the maintenance of resident charts and review documentation performed by caregiving staff.
  • Assess health, oversee medication administration, and manage individualized service plans for residents.
  • Communicate with families regarding residents' healthcare needs and scheduled appointments.
  • Participate in pre-admission screening of prospective new residents and ensure required documentation is completed.
  • Perform on-site evaluations of residents admitted to alternate care environments and maintain family contact.
  • Evaluate residents, document changes in condition, and notify relevant parties of any changes.
  • Facilitate continuity of care for residents receiving home health care and hospice services.
  • Attend and participate in weekly care conferences and staff meetings.
  • Ensure accuracy of medication administration records in the electronic health/medical record system.
  • Perform LOCA Assessments with assistance from the Executive Director.
  • Assist in maintaining a safe environment for associates to reduce Workers' Compensation claims.

Requirements

  • Valid RN/LVN license with a current Texas nursing license.
  • Experience in Assisted Living and Memory Care settings.
  • Education required to obtain state nursing license (RN/LVN).
  • Familiarity with Assisted Living regulations, especially regarding nursing services and medications.
  • Leadership experience is strongly preferred.
