About the position

The Director of Health & Wellness at our non-profit assisted living community plays a crucial role in overseeing comprehensive care services for residents. This leadership position ensures high standards of care, compliance with regulatory requirements, and directly impacts the quality of life for residents through proactive health management and meticulous oversight.

Responsibilities

  • Initiate resident service plans according to individualized needs prescribed by physician and/or community policy.
  • Manage the health care of all residents, disseminating information to families, staff, physicians, and third-party providers.
  • Perform on-site assessments of residents admitted to hospitals or skilled nursing facilities and maintain contact with families and discharge planners.
  • Complete pre-admission assessments for each resident and update every six months or as needed with changes in condition.
  • Meet with each resident regularly to provide health and wellness checks and answer health-related questions.
  • Review residents' medical reports prior to move-in to ensure community capabilities meet individual needs.
  • Monitor skin assessments of residents and assess wounds, obtaining physician orders for treatments and interventions.
  • Assist in achieving occupancy goals by monitoring residents' health, safety, and well-being, identifying risks for move-outs.
  • Schedule and follow through on ancillary medical services such as podiatrist, doctor visits, and dental visits.
  • Provide training, supervision, and monitoring of Medication Assistants in medication assistance, including frequent audits of medication sheets and passes.
  • Maintain ongoing communication with residents, families, physicians, and pharmacies regarding medication needs.
  • Assist the Director of Human Resources with new hire paperwork and monitoring time sheets.
  • Perform on-call duties for staffing and clinical needs, potentially assuming the role of Resident Assistant or Medication Aide as necessary.
  • Act as a key member of the Community's emergency response team during declared emergencies and disasters.

Requirements

  • Five (5) years of Assisted Living management experience.
  • Assisted Living Manager Certification.
  • Graduate of an accredited LVN program.
  • Current, unencumbered, active license to practice as an LVN in Texas.
  • Current CPR certification.
  • Demonstrated effective leadership skills in fiscal clinical management, strategic planning, program, and staff development.
  • Ability to work with and supervise others as an effective team builder and player.
  • Ability to evaluate performance and take corrective actions when necessary.
  • Ability to work autonomously, take initiative, set priorities, organize work, and make independent decisions.
  • Computer proficiency with electronic medical records and Microsoft Office suite, with the ability to learn new applications.
  • Effective communication skills with residents and colleagues.
  • Ability to speak, read, and write English.
