About the position

The Wellness Specialist at Concern Housing plays a crucial role in supporting individuals and families by providing linkages to community resources, daily living skills training, counseling, and ensuring compliance with program requirements. This position is essential for advocating for clients and facilitating their personal growth and independence through various support services.

Responsibilities

  • Facilitate residents' linkage with services available in the community (e.g., case management, clinical services, day programs, vocational services and opportunities, community integration, entitlements, etc.)
  • Development of comprehensive needs assessments and support plans
  • Meet with clients to develop skills and review progress
  • Responsible for all documentation required for client records (e.g., progress notes, discharge plans, etc.)
  • Provide individual counseling and support services
  • Provide medication management services
  • Engage in recovery planning through the use of Wellness, Recovery and Action Planning
  • Provide daily living skills training (e.g., medication monitoring and training, cleaning, money management, etc.)
  • Attend supervision and staff meetings
  • Remain current with all Agency required compliance documents and trainings
  • Other duties as assigned by the Team Leader

Requirements

  • Bachelor's Degree preferred in Psychology, Rehabilitation, Social Work or other related mental health discipline
  • 2 years experience working in a similar setting and/or working with people with psychiatric disabilities
  • Good verbal and written communication skills
  • Ability to make sound judgments regarding client care
