About the position

The Women Veteran Program Care Coordinator (WVPCC) Registered Nurse (RN) is responsible for providing high-quality, comprehensive health care services to women veterans. This role involves patient care, care coordination, health promotion, and education, ensuring that care is delivered in a sensitive and safe environment. The WVPCC RN collaborates with various healthcare providers and community programs to facilitate continuity of care and meet the specific needs of women veterans.

Responsibilities

  • Provide patient care, care coordination, health promotion, health education, and coaching on wellness, disease prevention, and chronic care management.
  • Formulate plans of care based on nursing assessments and Veteran goals of care.
  • Delegate elements of care to the appropriate individual based on assessed needs, condition, and safety of the patient.
  • Provide coordination of care across care settings, accessing healthcare providers and community programs, and communicating with patients and providers regarding evaluation and treatment plans.
  • Participate in outreach events to promote Women Veteran Program services.
  • Assist in meeting the Directive 1330.01 Health Care Services for Women and provide resources for Women Veterans.

Requirements

  • U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens.
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth of the United States.
  • Graduate of a school of professional nursing approved by the appropriate accrediting agency.
  • Proficiency in basic written and spoken English.

Nice-to-haves

  • Experience working in Women's Health.
  • Experience providing maternity care or gynecology care coordination.
