About the position

The Workforce Reentry Facility Case Manager at Eckerd Connects serves as the primary point of contact for participants in a correctional facility, focusing on recruitment, enrollment, and case management. This role involves facilitating group sessions and providing one-on-one support to help participants reintegrate into society and secure employment. The Case Manager is responsible for creating a positive environment, monitoring participant progress, and coordinating reentry transition planning.

Responsibilities

• Recruit and enroll program participants in the reentry program.
• Facilitate group sessions and conduct one-on-one case management.
• Confirm eligibility and manage enrollment activities, including scheduling intake sessions.
• Create and maintain an environment that reinforces appropriate behavior and positive morale.
• Evaluate and monitor participant progress on work assignments and develop lesson plans.
• Assist participants with job readiness, resume development, and skills training.
• Plan and coordinate events to help participants obtain and retain employment.
• Monitor release dates and coordinate reentry transition planning.
• Maintain accurate data in the Grantee Performance Management System.

Requirements

• Minimum Associate's Degree with relevant experience or Bachelor's Degree in Criminal Justice, Social Work, or related field (preferred).
• 3+ years of full-time experience working with justice-involved individuals or in peer support roles.
• Valid state driver's license and ability to meet Eckerd Auto Insurance requirements.
• Ability to pass a drug screen in compliance with workplace policies.
