About the position

The Reentry Facility Case Manager plays a crucial role in supporting participants within a correctional facility by providing case management services, facilitating group sessions, and assisting with job readiness and skills training. This position requires a proactive individual who can effectively manage participant enrollment and progress while fostering a positive environment that encourages personal and professional growth.

Responsibilities

  • Serve as the primary point of contact for participants in the detention facility.
  • Recruit and enroll program participants, facilitating groups and conducting one-on-one case management.
  • Confirm eligibility and manage enrollment activities, including scheduling and facilitating information and intake sessions.
  • Create and maintain an environment that reinforces appropriate behavior and positive morale.
  • Evaluate and monitor participant progress on work assignments and develop daily and weekly lesson/work plans.
  • Assist participants with NCWorks registration, job readiness, resume development, IDP development, skills training, and social skills.
  • Plan, implement, and coordinate events to enhance participant success in obtaining and retaining employment.
  • Adapt materials and equipment to meet specific needs and learning styles of participants.
  • Monitor release dates and coordinate reentry transition planning with the Program Manager and Community Case Manager.
  • Maintain accurate Grantee Performance Management System data, ensuring complete data entry according to established deadlines.

Requirements

  • Minimum Associate's Degree in combination with experience including lived experience; or Bachelor's Degree in Criminal Justice, Social Work, or related field (preferred).
  • 3+ years of full-time experience working with the justice-involved, peer support, or project management preferred.
  • Must be able to meet requirements for Eckerd Auto Insurance and drive for business purposes.
  • Must have appropriate and valid state driver's license.
  • Required to pass a drug screen in compliance with Substance Abuse and Drug-Free Workplace policy.
