About the position

The Wound Care Nurse is responsible for the effective management of the skin care program within the center, focusing on the treatment and healing of various types of wounds. This role involves assessing wounds, creating treatment plans, monitoring infections, and ensuring compliance with nursing standards and infection control protocols. The nurse will work collaboratively with other healthcare professionals to improve and maintain residents' skin integrity.

Responsibilities

  • Perform resident skin assessments and document findings to ensure skin integrity is improved or maintained.
  • Administer treatments as prescribed and in compliance with infection control protocols.
  • Coordinate and lead weekly wound rounds, including interdisciplinary care plan reviews and treatment recommendations.
  • Monitor the progress of wound healing and make recommendations for changes as appropriate.
  • Collect and analyze pressure ulcer information and maintain records of skin alterations.
  • Investigate and identify practices to minimize risks related to skin alterations.
  • Calculate and document weekly/monthly pressure ulcer rates.

Requirements

  • Graduate of an approved Licensed Practical/Vocational Nurse program and licensed in the state of practice.
  • One-year experience providing specialized treatments in a long-term care setting.
  • Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.
  • Current BLS/CPR certification.
