About the position

The Wound Care Nurse at Dumont Center for Rehabilitation & Nursing Care is responsible for the comprehensive management of residents with various types of wounds. This role involves coordinating with multiple departments to ensure effective treatment and care plans are in place, monitoring the response to treatments, and maintaining communication with residents' families regarding their care.

Responsibilities

  • Check all new admissions and readmissions for presence of wounds.
  • Coordinate with all Unit Managers and Nursing Supervisors for all nosocomial wounds.
  • Ensure that treatment orders are in place.
  • Monitor the response of the treatment orders.
  • Conduct weekly wound rounds with the Wound Doctor.
  • Initiate individual wound care plans.
  • Update and distribute weekly wound tracking to other departments.
  • Evaluate and update wound care plans quarterly or with significant changes.
  • Inform Next of Kin of residents with wounds upon admission, readmission, and in-house developments, documenting in progress notes.
  • Ensure residents with wounds have specialized mattresses and off-loading devices.
  • Coordinate with the Rehab Department for positioning devices.
  • Coordinate with the Dietician for nutritional needs.
  • Coordinate with Attending Doctors for blood values and changes in treatment orders.
  • Ensure treatments are done and specialized mattresses and off-loading devices are in place in collaboration with Unit Managers and Supervisors.
  • Conduct weekly audits for off-loading devices and special mattresses, reporting findings to the Director of Nursing.
  • Assist the Nursing Department as needed per directives from the DNS or ADNS.

Requirements

  • Registered Nurse with at least 2 years of experience in wound care.
  • Ability to develop and implement care plans for residents with wounds.
  • Strong communication and coordination skills with various departments.
