About the position

The Wraparound Facilitator at Eckerd Connects is responsible for facilitating a high-fidelity wraparound process for youth and families, providing individualized, coordinated, family-driven care to meet the complex needs of youth involved with multiple child and family serving systems. This role requires extensive travel and a flexible schedule, including nights and weekends, to ensure comprehensive support for families at risk of out-of-home placement due to behavioral and mental health challenges.

Responsibilities

• Facilitate a high-fidelity wraparound process for youth and families.
• Provide individualized, coordinated, family-driven care to meet the complex needs of youth.
• Coordinate a collaborative process involving youth, family, providers, and social support networks to build a plan of care.
• Work with an Independent Evaluator to ensure comprehensive assessments are completed and incorporated into the CFT process.
• Develop an individualized Plan of Care in partnership with the family, adhering to National Wraparound Initiative standards.
• Assist families in accessing strength-based mental health, social services, educational services, and other necessary supports.

Requirements

• Bachelor's degree in social, human services, or behavioral sciences required.
• Minimum of two years of full-time experience in relevant family, children/youth, or community service capacity.
• Master's degree preferred.
• Obtain wraparound practitioner certificate within 2 years of hire date and maintain it, including annual re-credentialing.
• Must meet requirements for Eckerd's Auto Insurance and be able to drive for business purposes.
• Ability to provide 24-hour, 7-day a week on-call crisis intervention to youth.

Nice-to-haves

• Experience working in a relevant family, children/youth, or community service capacity if degree is in a field other than human services.
