About the position

The Disease Management Coordinator for Ambulatory Operations at West Virginia University Hospitals is responsible for collaborating with patients and primary care providers to ensure the delivery of quality, efficient, and cost-effective healthcare services. This role involves coordinating, monitoring, and evaluating healthcare options to optimize patient health status, while also facilitating transitions of care and managing resources effectively.

Responsibilities

  • Collaborate with patients and primary care providers to ensure quality healthcare services.
  • Coordinate, monitor, and evaluate healthcare options for patients.
  • Identify targeted populations and risk stratify patients to prioritize needs.
  • Communicate with inpatient and outpatient case management to implement discharge plans.
  • Work with physicians, patients, and families to ensure safe transitions of care.
  • Design individualized care plans in collaboration with patients.
  • Coordinate with social services for resource and financial management.
  • Facilitate patient progression throughout the continuum of care.
  • Engage in clinical performance improvement and quality activities.
  • Use data to drive decisions regarding patient care.

Requirements

  • Current unencumbered licensure as a registered professional nurse with the WV Board or appropriate state board.
  • Associate's Degree in Nursing with five years of clinical experience, or Bachelor's Degree in Nursing with three years of clinical experience.
  • West Virginia state criminal background check required.

Nice-to-haves

  • Bachelor's degree in Nursing (BSN) preferred.
  • Three years of disease management experience preferred.
