About the position

The Youth Case Manager for the Nia Program at NorthPoint Health & Wellness Center is responsible for collaborating with black youth aged 12 to 24 to create individualized plans that promote school success, life skills, physical health, and a sense of purpose. This role emphasizes a holistic approach to youth services, focusing on integrated care and addressing social and health inequities within the community.

Responsibilities

• Collaborate with youth to develop individual plans for school success and life skills.
• Facilitate group empowerment activities and programs for youth.
• Coordinate with community resources to support youth needs.
• Advocate for youth and address social and health service needs.
• Implement creative solutions to engage youth and their families in programs.

Requirements

• Associate degree in human services or related field with three years of experience in case management or care coordination.
• Bachelor's degree in human services or related field preferred.
• Minimum two years of case management experience or youth development programming experience.
• Experience working with justice-involved youth and knowledge of trauma impact.
• Proficient in MS Word, MS Excel, and client tracking databases.

• Strong organizational skills to coordinate youth service programming.
• Ability to communicate effectively and build trust with youth.
