About the position

The Youth Community Specialist (YCS) plays a crucial role in providing case management and community psychiatric rehabilitation services to at-risk youth facing serious emotional disturbances. This position focuses on developing resiliency goals, teaching self-advocacy, and engaging families and support systems to promote holistic health and wellness. The YCS is responsible for direct service delivery, crisis support, discharge planning, and maintaining documentation in compliance with confidentiality requirements.

Responsibilities

  • Develop resiliency goals and identify strengths, needs, abilities, preferences, skills, resources, and supports.
  • Teach youth how to self-advocate and access resources to assist in developing resiliency and promoting holistic health.
  • Provide direct services through educating, modeling, and teaching skills.
  • Engage youth, families, schools, and other supports in developing person-centered treatment plans.
  • Support youth in crisis to access necessary treatment services.
  • Assist in discharge planning after hospitalizations or out-of-home placements.
  • Manage time effectively and attend required meetings and documentation deadlines.
  • Document all services provided in the electronic health record, ensuring confidentiality and compliance with HIPAA.
  • Maintain records of consumer contacts for reporting and quality improvement.
  • Treat youth and families with dignity and respect, maintaining good relationships with referral sources and other departments.

Requirements

  • Bachelor's degree in social work, psychology, or a related field, or equivalent experience (one year of experience for each year of required education).
  • Knowledge of trauma-informed treatment and practices.
  • Understanding of behavioral health, including diagnosis and intervention strategies.
  • Familiarity with the juvenile justice system, Division of Family Services, and educational systems.
  • Excellent verbal and written communication skills.
  • Ability to work independently and use sound judgment.

  • Knowledge of youth mental health diagnoses and treatment practices.
  • Experience with community resources and the ability to develop new resources.
  • Empathy skills and the ability to develop rapport with diverse individuals.
