About the position

We are seeking a dedicated YDHP Coordinator to join our team at TAY. In this position, you will work closely with former foster youth and unhoused youth, ages 15-24, providing case management, guidance, support, and advocacy to help them navigate the challenges of transitioning to independent living and adulthood. As a YDHP Coordinator, you will provide case management services to a caseload of clients, which includes completing intakes, goals sheets, updating data in the Homeless Management Information System (HMIS), and serving as a Connector for the Coordinated Entry System. You will create a warm, welcoming, and safe space for youth in the program, establishing and maintaining close working relationships with them. Advocacy is a key part of this role, as you will serve as a positive role model and ensure the safety and security of the facility in a trauma-informed manner. Additionally, you will assist with household duties such as meal preparation, shopping, laundry, and cleaning to maintain a welcoming environment. Co-facilitating group activities and providing transportation as needed will also be part of your responsibilities. You will lead weekly workshops and participate in weekly outreach activities, assisting youth in building independent living skills, including housing, employment, and money management.

Responsibilities

  • Provide case management services to a caseload of clients, including completing intakes and goals sheets.

Requirements

  • B.A. in related field such as sociology or psychology.
  • At least two years' work experience with young adult populations, preferred.
  • Strong knowledge of professional ethics required to work with vulnerable populations.
  • Strong professional writing and oral communication skills.
  • Strong, culturally competent and respectful interpersonal communication skills.
  • Must have a vehicle, valid CA driver's license, vehicle insurance, and good driving record.

Nice-to-haves

  • Bilingual Spanish-speaking ability, preferred.
